School Nurse

Health History

Families must complete a Health History Form when their child enters 4K. Our school nurse reviews each form and compiles a list of health concerns by grade level. This information is shared with staff to help us provide the best care for your child during the school day.

If your child’s health changes at any time, please contact the school office or nurse as soon as possible.

Physicals 

Students attending a daycare center 4K site are required by law to have a physical exam on file. Additionally, all students entering 5K must complete a physical exam, as well as an eye and dental exam. Forms for these required 5K exams are mailed home at the end of the 4K year and are also available on the District’s health services webpage.

Immunizations 

Under Wisconsin state law, schools must ensure that all students are either up to date on their immunizations or have a signed waiver on file. Families are responsible for providing written proof of immunizations. The school must receive the first date of each required vaccine by the 30th day of school, the second by the 90th day, and any remaining dates by the 30th day of the following school year. Failure to comply may result in court action and a fine of up to $25 per day of violation, as enforced by the Jefferson County District Attorney’s Office.

Medication 

All medications, including prescription medications, must be kept in the school office in a locked cupboard unless they are emergency medications (such as inhalers) approved to remain with the student. Medications must be brought to school in the original pharmacy or manufacturer’s container and accompanied by a completed medication form.
